The Elevation Model
The Elevation Model of Community Health Improvement is an action model for creating effective and sustainable community health improvement projects. It can be used by anyone engaged in community health improvement to develop a new project or strengthen an existing project.
In the Elevation Model the community, service providers, and investors work together to generate sustainable investment in effective practices which improve the health status of a defined group of people. This work may require any or all of the core competencies identified in the model.
Community Health Solutions equips people to use the Elevation Model through our Elevation Platform. We work closely with our clients to produce customized solutions as illustrated in our Elevation Projects.
